Feasibility of Care Coordination Study

In April 2018, recruitment began for a pilot study within the Duke Integrative Primary Care (DIPC) clinic.  This study will explore the feasibility of incorporating a nurse primary care coordinator role into the operations of the clinic. During the study, a nurse care coordinator will contact a randomly-selected cohort of DIPC members and offer individualized health support several times over the course of 6 months.
